Application for Award of Graduate Degree Form

DUE to GAPE by the posted deadline . Do not send form to the iSchool. 

  • Due date for May graduation: February 10
  • Due date for August graduation: June 10
  • Due date for December graduation: September 10
The Application for Award of Graduate Degree Form is how you apply to SJSU for graduation after your candidacy has been approved.

Filling out the Form:

Signature: Sign electronically or just type in your name

Planned Graduation Date: Use just the month and year.  For example: December 2012.

Major and Concentration: MLIS

Degrees Now Held: Leave blank or list bachelor's degree and any other degree you might already have

How to Submit:

Regular and Special Session students:
Emailing the form in PDF is the best way to submit and will ensure a receipt confirmation: Email: Cristina Velarde.
