Application for Award of Master's Degree Form

DUE to GAPE by the posted deadline (for December 2016, due September 10, 2016; for May 2017 graduation, due February 10). Do not send form to the iSchool.

The Application for Award of Master's Degree Form [PDF: 2 pages] is how you apply to SJSU for graduation after your candidacy has been approved.

Filling out the Form:

Signature: Sign electronically or just type in your name

Planned Graduation Date: Use just the month and year.  For example: December 2012.

Major and Concentration: MLIS

Degrees Now Held: Leave blank or list bachelor's degree and any other degree you might already have

How to Submit:

Regular and Special Session students:
Emailing the form in PDF is the best way to submit and will ensure a receipt confirmation: Email: Cristina Velarde.
