University Organization Agreement
Based on a directive from the California State University (CSU) Chancellor’s Office we need to have an internship agreement signed and on file with all of our internship sites. The agreement will have a 5-year lifespan.
Please complete the following four steps
- Step One: Register yourself and your organization in the School of Information internship database (you only need to do this once). See instructions on the Internship Site Supervisor Help page.
- Step Two: Set up and submit your internship via this site supervisor login page in the School of Information internship database. Be sure to hit the submit button after you set up the internship. The internship will automatically go into in a pending state until you submit the University Organization Agreement.
- Step Three: After you have completed steps one and two you will receive a link from the University to review, sign, and return the University Organization Agreement (UOA). This link will come to your email and you will sign it electronically. Be sure to check your spam filter.
- Step Four: Send an email confirmation. If you sign and return the electronic agreement with no changes please let us know by e-mailing a confirmation to Kristina Luna (Kristina. Luna@sjsu.edu) and Linda Main (Linda.Main@sjsu.edu).
Upon receipt of your email we will change the status of your internship from pending to active in our internship database.
The University often takes a long time to process agreements but if we know you have signed the agreement with no changes we will activate your listing immediately.
If you need to make changes to the agreement the agreement will be routed to our Contracts department and the internship listing will remain pending.
